Warning! This is definitely non-canonical. This would be an alternate universe story!
The Year is 1923. Doctor John Hamish Watson is now dead, under some suspicious circumstances. In his place, Mary Morstan Watson has been helping Sherlock Holmes on his cases.
During a case in New York City in which Harry Houdini was involved, Holmes has for some reason vanished. Mary returns to England and begins a yearly ritual of straightening her papers, discarding what she no longer needs and filing the rest.
She discovers that a blue Delft porcelain pellet was removed from Doctor Watson’s body. It reminds Mycroft of one taken from the body of a woman named Gabriela Paraskeva, a spy. The pellet was coated in ricin, and fired into her body using an air gun concealed within an umbrella. It was simply a matter of walking up to her on the street and brushing against her to fire the pellet. The ricin was released slowly and she died over a period of several days.
The pellet from Watson’s body is identical to the one from the lady spy.
Mary takes it upon herself to investigate the death of her husband. It will take her to South Africa to visit the Delft Porcelain factory and to Germany to visit the Metzger Chemische, that manufactures castor oil and medicinal ricin, but the dregs from the process make the deadly form of ricin.
Mary seems to be brushed off at every question, finally receiving a message that: “Some events and the people involved in them are best left undisturbed.”
The story winds through unrecorded cases of Holmes and Watson, through the dealings of an unnamed group of men, and through danger and false friends. The mystery is good. It is just that it never really gets an answer.
There are places where the prose flows well and places where it drags. In the end the major let down is that there is no clear answer as to what happened. Watson has been murdered. Who, how, and why remains a mystery.

Alternative Universe Holmes story.

This is an alternative universe Holmes story where Sherlock Holmes barely appears and perhaps that is why there seems no clear-cut resolution to the question "What did John H. Watson M.D. die of?" His wife Mary Morstan Watson finds his forgotten death certificate and realizes that under the cause of death there is only the notation "indeterminate". She consults with the doctor who performed the autopsy and is told there was an odd pellet found in his body at that time. This leads her on a months'-long trip to South Africa and other places to track down the pellet's origins and murderous use. If you like Sherlock Holmes stories with no Sherlock Holmes and yet another version of what Mary Watson might have been like, try "Mary Watson and the Departed Doctor."
